Most patients when they come for a consultation are familiar with the use of hyaluronic acid dermal fillers. Brand names which are well known include Juvederm, Restylane, Teosyal and Stylage however there are over 200 hyaluronic acid dermal fillers available in Europe which carry the CE Mark. Hyaluronic acid is a long chain sugar molecule which is naturally occurring in the skin, the joints and in the eyes but can also be used to improve the appearance of the skin.
Many patients who come specifically for this type of treatment already know that they would like to enhance their lips for example, or the cheekbones, chin or treat areas of the face which have become volume depleted with ageing. Others would prefer to use they hyaluronic acid as a restorative agent, creating improved skin tone and texture with products such as Restylane Vital, Teosyal Redensity 1 and Juvederm Hydrate which are used to improve fine lines and wrinkles, and the luminosity of the skin. A new hyaluronic acid available in Europe form Italy is Profhilo, developed by Professor Chiara Schiraldi and Professor Mario de Rosa in the Second University of Naples. What makes this new product so exciting is its ability to improve areas such as the neck and forehead, which can be difficult to treat using more traditional hyaluronic acids. This is because of the way Profhilo is able to flow freely through the tissues without causing lumps or bumps in these tricky regions. Profhilo is able to spread through the tissues because of its new method of linking the chains of hyaluronic acid together without the use of 1,4-butanediol diglycidyl ether (BDDE), using instead a heat method. In addition this hyaluronic acid dermal filler combines both long and shorter chains of hyaluronic acid has been shown in studies to improve the wound healing and formation of new collagen and elastin production with minimal tissue inflammation.
Skin laxity is a difficult problem to treat and it can be difficult to use lasers, surgery or strong chemical peels. I find this product very useful in my office because of the minimal downtime associated with it. The procedure is very simple. A very fine needle is used on a syringe connected to the dermal filler, which comes in a 2ml syringe, to apply the hyaluronic acid dermal filler just underneath the surface of the skin. Unlike traditional dermal fillers, Profhilo is injected in a bolus or ball at strategic points in the face and neck, depending on the areas to be treated. As Profhilo can spread so effectively through the tissues, there is no danger of it causing a lump in the areas it is injected, instead if flows and gives a supportive layer of hyaluronic acid under the skin. There is no need for any local or topical anesthesia with this treatment as discomfort is extremely minimal. I have found that there is on occasionally a small bruise at a single injection point, which is always a risk when performing a treatment of this nature. Treatment sessions are 1 month apart with at least 2 sessions necessary for the best results.
In comparison with other biostimulatory products such as Restylane Vital or Teosyal Redensity 1 there are fewer injection points necessary with Profilo and treatment sessions are a longer time apart. With results lasting around 4-6 months per treatment it is easy to see why this treatment has rapidly gained traction with my patients both as a standalone application or with other treatments such as wrinkle relaxing injections or other hyaluronic acid dermal fillers.

I have also been using the new Juvapen over the past few months to administer Botox anti wrinkle injections and it has been a very positive experience for me. The benefits of the pen in allowing micro doses of 0.5U botulinum toxin have allowed me to create a much more refined treatment result which is difficult to achieve by hand as the dosing is so small.
